Merrill Gardens - 2600 North Milwaukee Street - Responsibilities: Develop activities collaboratively with the Active Living Program Director for residents transitioning from Assisted Living to Memory Care; Deliver, lead, or conduct program activities in a timely and effective manner; Coordinate at least one family activity per month; Observe residents abilities related to daily ADLs and social skills and communicate opportunities or difficulties to the team; Participate in all employer-provided training and assume duties of Activities staff in their absence
