Social Work Position at Boise VA Medical Center

This full-time, permanent social work position is located at the Boise VA Medical Center (BVAMC) within the Geriatric and Extended Care (GEC) Service Line of Nursing Service. The incumbent has broad clinical responsibilities as a key member of the Home-Based Primary Care (HBPC) treatment team at the BVAMC.

Responsibilities include:

• Providing a comprehensive psychosocial assessment and integrating it into an overall plan of care in collaboration with the interdisciplinary treatment team, the veteran, family members and significant others, whenever possible.
• Developing a treatment plan with the veteran based on this assessment.
• Addressing the unique needs of homebound, frail, disabling/chronic and/or dying veterans.
• Assisting program director and medical director in reviewing new consults for admission to the HBPC program.
• Participating in weekly interdisciplinary care planning meetings.
• Acting as the community liaison, providing education to the community resource and health care providers.
• Providing leadership, direction, orientation, coaching, in-service training, staff development and continuing education programs for assigned social work staff.
• Coordinating community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
• Providing education related to VA and community resources, entitlements, Advance Directives/Living Will and referring veterans/families to the appropriate interdisciplinary team member for identified health education needs.
• Independently identifying and prioritizing high-risk patients and providing case management services as needed.
• Providing crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinating family conferences and serving as liaison to family members.
• Utilizing advanced practice skills, professional judgment and expertise regarding the veteran's self-care capability, family support system, and the veteran's health care needs and possible consequences.

Work Schedule: 8:00am to 4:30pm Monday to Friday

Qualifications:

Basic Requirements:

United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.

English Language Proficiency Requirement: No person will be appointed under authority of 38 U.S.C., chapter 73 or 74, to serve in a direct patient-care capacity in VHA who is not proficient in written and spoken English.

Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).

Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level.

Grade Determinations:

Senior Social Worker GS-12 Experience and Education: The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level.

Demonstrated Knowledge, Skills, and Abilities:

Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.

Preferred Experience: Experience in medical social work, primary care and case management.
