Candidate will provide de-escalation, stabilization, and referrals to higher levels of care to individuals experiencing a mental health and/or substance use crisis across Terry Reilly clinics. This position may travel across clinics based on patient need.ESSENTIAL DUTIESRespond to incoming contacts and requests for crisis support via telephone, telehealth, or in person. Complete outbound call requests with accuracy, compassion, and efficiency.Demonstrate effective engagement and crisis intervention skills, de-escalation techniques, and stabilization methods alongside interdisciplinary team.Provide phone consulting and triage needs for callers with a behavioral health emergency. Screen incoming calls for safety and collaborate with the Patient and/or other Terry Reilly staff in an effort to reach crisis resolution. Utilize Motivational Interviewing and provide brief solution-focused interventions to patients while assessing current level of need with patients awaiting a higher level of care or provider placement, including but not limited to safety and crisis response plans and general case management duties. Effectively utilize screening tools and risk assessments based on acuity and risk and complete referrals for high acuity cases requiring additional intervention, such as hospitalization, police, or mobile crisis response.Knowledge of and competency with computer skills to be able to search and enter data in the EHR system.May assist in maintaining registry of patients who score moderate and high risk on CSSRS and provide appropriate outreach, including conducting a risk assessment and/or crisis response plan, as needed.Maintain a presence at the Terry Reilly walk-in clinics to assist in monitoring and completing BH intakes, as well as providing intervention for case management staff meeting with high acuity patients.Meet with behavioral health and SUD patients identified as a hospital discharge to complete a warm hand off and engage patient in services that eliminate barriers in social determinants of health (i.e.- food insecurity, transportation limitations, lack of insurance coverage, etc). Actively engage in and participate with the Crisis Intervention Team Collaborative committee, as well as partner with internal Terry Reilly employees working to enhance community partnerships.MINIMUM QUALIFICATIONS Successful clearance of Department of Health and Welfare criminal background check. Bachelor's Degree (psychology, social work, counseling, or related field) with existing credentialing from Crisis Prevention Institute (CPI) or an equivalent Crisis Intervention certification, or ability to obtain within 90 days of employment. 2 years working in a triage or crisis unit capacity with experience in crisis intervention and suicide prevention/intervention/postvention. Experience collaborating with disenfranchised patient populations, including psychiatric and substance use populations. Critical thinking and effective decision making for managing high-acuity crisis requests and decision-making skills that facilitate safety and crisis resolution.PREFERRED QUALIFICATIONS Master's Degree (psychology, social work, counseling, or related field) and active Crisis Intervention certificationBehavioral Health professional or Licensed Clinician (LPC, LCPC, LSW, LMSW, LCSW, QP) Experience in crisis call centers, or community crisis centers, or mobile crisis units Peer Support Specialist certification, or a Certified Recovery Coach Trained in Applied Suicide Skills Training (ASSIST) or Safe Talk Bilingual in Spanish and English Experience working in a medical setting, community health setting, or as a member of a multidisciplinary team. Knowledge of computer and email systems, as well as EHR. Knowledge of community resources, including those for the migrant and farm worker populations. Knowledge of Patient Assistance/Medication Assistance Programs. Familiarity with State of Idaho Health & Welfare Mental Health system.
